Looks like ejecting at 2 o-clock position which I have read is a sign of over gassing. just built my ar pistol with a adj gas block so I'll see how it goes. Should be adjusted to eject at the 3-4 o-clock position. Great looking gun.
Why it happened? Radical BCG’s are trash.. Change that out and keep everything else
Nice job taking everyone through your thought process during the cleaning. Even if you are an amateur shooter/gunsmith like myself, it usually just takes a few minutes of thinking through the processes to find the problem. If I had a similar shooting experience I would literally do the same thing, take it home, pull it apart and work through the problem.
Parts break, shit goes wrong, just take it a step at a time and figure it out
Did you nail down the reason why the failure happened in the first place? Just a faulty extractor? Or was your gun over-gassed? Anyone give you any good insights? I read through some of the comments and there are a million explanations, figured I would just ask you directly

Rule of thumb always carry spare parts for your bolt extractor.
Just happened to me the other day. The exact same break line. I clean mine regularly, but I do use cheap ammo. So I'm guessing that's my problem. Or just that my ar is cheap
They wana talk shit cause you run a Sightmark RDS oh well fuck em…to be so cheap that RDS holds its own right up there with EOTech, Aimpoint and Vortex. I run a Sightmark on my 300blk with well over 1000 rounds and have had 0 issues or complaints. Solid piece of equipment

I had the same exact problem last Sunday with my AR “pistol”. Only after about 450 rounds both claws broke on the extractor. Same exact problems you had. My AR was super clean too. I’m gonna buy a full bcm bolt carrier group I think to replace the current.
